Release of Liability
I, the undersigned, have voluntarily enrolled myself and my dog(s) to participate in services offered by Major Wags Dog Training, LLC. Major Wags Dog Training, LLC endeavors to create as safe a training and/or boarding environment as possible, however I acknowledge that participating in dog training and/or boarding services involves certain risks including, but not limited to, injuries, death, property damage, or other harm to me, my dog(s), or other dogs. I accept and voluntarily assume all risks of any bodily injury, death, or property damage arising out of or related to my participation in a Major Wags Dog Training, LLC service.

I hereby agree for myself and for all my heirs, executors, administrators, successors, and assignees to (i) forever release, waive, and/or discharge Major Wags Dog Training, LLC of all liability, actions, causes of action, claims, and demands for any and all loss, suffering, or damage I may have and any claims or demands I may have on account of injury to my person or property (including, but not limited to, my dog(s) or my person, dog(s) and/or property of others, including death) arising out of or related to my dog(s) participation in any of the training programs and/or boarding programs provided by Major Wags Dog Training, LLC and (ii) indemnify, defend, hold harmless, and reimburse Major Wags Dog Training, LLC from any claims, including attorney's fees, that Major Wags Dog Training, LLC may incur arising directly or indirectly out of my dog(s) participation in any of the training programs and/or boarding services provided by Major Wags Dog Training, LLC.
Acknowledgement of Limitations

I agree that the recommendations made by Major Wags Dog Training, LLC of any product is not a guarantee of performance or my satisfaction with the product. I acknowledge that breeding, temperament, cognitive ability, and social conditioning play a role in my dog(s) psychology and may affect my dog(s) ability to learn and achieve training objectives. Furthermore, although Major Wags Dog Training, LLC shall use commercially reasonable efforts to achieve a sound level of obedience, control, and behavior modification of my dog(s), I acknowledge and agree that there is no 100% guarantee that my dog(s) will achieve the desired level of training or be cured of its behavior issues, and once services have been rendered I do not qualify for a refund based on subjective dissatisfaction alone. I also acknowledge the important role that I play in my dog's training, and that it is my responsibility to continue working with my dog daily through the program objectives to maintain training progress.
I understand that a variety of tools and techniques may be used to train and modify the behavior of my dog(s), including but not limited to: crate, harness, squirt bottle, Pet Corrector (compressed air), slip lead, muzzle, flat buckle collar, martingale collar, dominant dog collar, prong collar, bark collar, electronic collar. I acknowledge that it is my responsibility to use the tools as designed, and apply the techniques as I have been taught, to achieve the desired outcome.
Permission To Use Images
I hereby assign and grant to Major Wags Dog Training, LLC the right and permission to use and reproduce, distribute, and publish the photograph(s), video(s), audio recording(s), and electronic representation(s) made of me and/or my dog(s) at any time during the training and/or boarding of my dog(s). I further release Major Wags Dog Training, LLC from all liability resulting from such use, reproduction, distribution, and publication. I hereby waive any right to seek any compensation for any of the foregoing.
Health and Vaccinations
I hereby certify that my dog(s) is in good health and has not been ill with any communicable diseases (within a 30-day period) prior to participating in any program offered by Major Wags Dog Training, LLC. I further certify that my dog(s) is current (within 12 months of participating in any Major Wags Dog Training, LLC program) on standard annual vaccinations (DHPP, Bordetella, and Rabies) and free of intestinal parasites (within 6 months of participating in any Major Wags Dog Training, LLC group or boarding program).
In the case of a medical emergency, I authorize Major Wags Dog Training, LLC, in its sole discretion, to seek veterinary care for my dog(s), and to make medical decisions, at a veterinary hospital of Major Wags Dog Training, LLC's choosing, at my sole cost and expense. Major Wags Dog Training, LLC agrees to notify me immediately upon seeking veterinary care for my dog(s) and I agree to immediately reimburse Major Wags Dog Training, LLC for all expenses (including, but not limited to, veterinary, medical, and cleaning expenses) incurred because of any of my dog(s) injuries and/or illness.
Payment and Refund Policy
I acknowledge that all services must be paid for, in full, to secure my place on the Major Wags Dog Training, LLC calendar. I understand that services will not be rendered until payment is complete.
Training Programs:
I understand that training programs have an expiration:
- Introductory Lessons, Single Private Lessons, and Private Lesson Packages expire 4 months from purchase date.
- Day Train Programs (and their included Follow-Up Lessons) and Board and Train Programs (and their included Follow-Up Lessons) expire 12 months from purchase date.
I acknowledge that if I need to cancel or reschedule a lesson that has already been booked and paid for, I must provide at least 24 hours’ notice to avoid any penalties. Failure to provide 24-hour notice may result in the forfeiture of the session, and I understand that no refund will be issued.
Additionally, I understand that arriving more than 15 minutes late to a scheduled lesson may be considered a missed session and could result in forfeiture without refund.
Exceptions may be granted in cases of family emergencies, medical issues, natural disasters, or public health crises, at the sole discretion of Major Wags Dog Training, LLC.
General Boarding:
Once a boarding reservation has been booked and paid for, I acknowledge that a $25 rescheduling fee will apply to each request to modify the reservation dates, up to 7 days prior to the scheduled start date. I understand that no modifications will be accepted within 7 days of the reservation start date.
Additionally, I understand that if I cancel my reservation more than 7 days before the scheduled start date, the amount paid will be credited to my account for future use. Cancellations made within 7 days of the start date are non-refundable.
Exceptions may be granted in cases of family emergencies, medical issues, natural disasters, or public health crises, at the sole discretion of Major Wags Dog Training, LLC.

Agreement to Terms and Conditions
I have carefully read and agree to the terms and conditions above. This agreement supersedes all prior discussions, representations, or agreements and expresses the entire agreement between Major Wags Dog Training, LLC and me. I confirm that except for that which is specifically written in this agreement, no promises, representations, or oral understandings have been made. This agreement has no time limit and is valid and enforceable for any and all programs for while I participate with Major Wags Dog Training, LLC.
